What an open seat really costs

What does an open seat cost you every week?

An open role costs you more than the recruiting fee. Someone covers the work, waits for care, or stops a service. We ask what is falling behind and who is carrying it. Use your own figures to compare the cost of waiting with the cost of hiring.

01

The seat is costing you every week.

Overtime, diverted patients, a stretched team and a schedule held together by favors.

02

The posting brought the wrong people.

Applicants arrive, but the person who has done this exact work is employed and not looking.

03

You are the one chasing.

Screening calls, no-shows and follow-ups land on the desk of someone with a facility to run.

The work behind the title

What does this hire really come down to?

Integrated behavioral health in an FQHC is not a traditional therapy caseload. The LCSW may move from scheduled follow-up visits to a primary care clinician’s same-day request, then return to charting, care coordination, and patient outreach. You need a clinician who can make sound scope decisions in short encounters.

Listen for a focused assessment, a brief intervention, a clear next step, and communication back to the care team. A candidate who expects every concern to begin with a full intake may struggle with this workflow.

Also test how they set clinical boundaries. The role may address stress affecting diabetes care, depression screening follow-up, medication adherence concerns, or a behavioral health referral. The LCSW must know when brief work fits, when scheduled follow-up is appropriate, and when a patient needs another level of behavioral health support. Your clinical leaders make final decisions on care protocols and scope.

Screening

What do we check before you see anyone?

Confirm an active LCSW license in your state. We review stated license status and work history, while you complete license verification, credentialing, and your own clinical assessment. Ask whether the candidate has practiced under the billing and supervision structure your clinic uses.

  • Ask for a recent example of a same-day referral from primary care. What information did they need from the medical clinician, what could they address that day, and what follow-up did they arrange? This shows whether they can work from a short handoff rather than a lengthy referral packet.
  • Discuss brief intervention methods the candidate has actually used, such as Cognitive Behavioral Therapy or Acceptance and Commitment Therapy. Ask how they would adapt one technique for a 15- or 30-minute visit without turning the conversation into a full therapy session.
  • Review charting habits in an electronic health record. Ask how they document the presenting concern, intervention, plan, and communication with primary care. Explore their familiarity with documentation that supports the clinic’s billing processes and HRSA-related reporting work.
  • Ask about daily visit volume without treating one number as a universal test. A clinician who has only carried weekly, hour-long appointments may still fit, but should explain how they would manage interruptions, same-day referrals, notes, and scheduled patients.
  • Test team communication with a concrete scenario. If a primary care clinician asks for input before the patient leaves, how would the LCSW share relevant information, protect appropriate privacy, and state the next behavioral health action? Listen for concise, usable communication.
Sourcing

Where do we find them, and why would they move?

We often look at clinicians from community health centers, public health systems, and outpatient behavioral health programs. Some may consider a move when their current role offers little contact with primary care, unclear referral practices, or a caseload that does not match their preferred clinical focus.

Private practice and group-practice clinicians can also be relevant. They may seek regular collaboration and a broader patient mix, while others may decide that brief integrated visits are not for them. We surface that distinction early instead of presenting a conventional therapist as an automatic match.

Avoid these

Which mistakes stall the search?

A common mistake is selecting for long-term psychotherapy depth without asking how the clinician structures a brief same-day intervention. Another is leaving the handoff process vague until late interviews, then learning the candidate expects protected appointment blocks. The key distinction is between outpatient psychotherapy experience and integrated primary care judgment.

03

Treating documentation as clerical work instead of testing how the candidate links the note, care plan, billing process, and team communication.

Role-specific search plan

How would we run the search for this opening?

Tell us how scheduled visits, warm handoffs, team huddles, documentation, and follow-up calls fit into a normal day. We discuss licensure, relevant work history, short-visit methods, EHR use, referral judgment, and reasons for considering a move. Meet candidates with a primary care leader and behavioral health leader when possible.

Questions

How do we assess a candidate who is relocating?

Confirm when the candidate can hold an active license in your state and complete your hiring process. Ask about their move timeline, existing obligations, and willingness to begin only after your credentialing steps are complete. Ask what the candidate does when a physician introduces a patient during a medical visit.

Do we need an LCSW with previous FQHC experience?

It can shorten orientation, especially when your clinic uses specific reporting or billing practices. It is not the only route to a strong hire. We also assess related integrated-care, community clinic, and public-system work.

How should we discuss a counteroffer with an LCSW considering integrated primary care?

Ask why they considered leaving before discussing a response to the counteroffer. If their concerns involve visit structure, team communication, or career direction, compensation alone may not settle the decision.

What does permanent placement cost?

No upfront fee. You pay only if you hire. We offer a candidate replacement guarantee on standard search agreements. The applicable period and exact terms are set out in the signed client recruiting agreement.

Can you help with coverage instead of a permanent hire?

We arrange locum tenens, travel and contract coverage on request. Credentialing, licensing and malpractice coverage are confirmed per engagement before anyone starts.

Next step

What should you send us about this role?

Send us the role, where the person will work and what needs to change with this hire. We will ask about the work, not just the title. Share any license or schedule limits that could rule someone out before we begin the search.

If you need an LCSW who can work beside primary care without losing clinical judgment, Healthcare Headhunters can start the permanent search with you.

No upfront fee. You pay only if you hire. We offer a candidate replacement guarantee on standard search agreements. The applicable period and exact terms are set out in the signed client recruiting agreement.

How long will it take to see a short list?

That depends on the job, schedule, available people, and what your employer can offer. We discuss those constraints before outreach and update you as candidates respond. We do not promise a fixed shortlist date. We check the job brief before arranging interviews. Tell us which parts of the role you can change.

What are the replacement terms?

We offer a candidate replacement guarantee on standard search agreements. The applicable period and exact terms are set out in the signed client recruiting agreement. Ask us to walk through those terms before you sign, so you know how a replacement would work for your opening and who would start it.

What do we not do in a standard permanent search?

We do not bill standard contingency search by the hour or simply repost your job. Temporary coverage is a separate arrangement. Any retainer or different engagement must be agreed, not assumed. Tell us early if you need coverage while the permanent search runs.
