What an open seat really costs

What does an open seat cost you every week?

An open role costs you more than the recruiting fee. Someone covers the work, waits for care, or stops a service. We ask what is falling behind and who is carrying it. Use your own figures to compare the cost of waiting with the cost of hiring.

01

The seat is costing you every week.

Overtime, diverted patients, a stretched team and a schedule held together by favors.

02

The posting brought the wrong people.

Applicants arrive, but the person who has done this exact work is employed and not looking.

03

You are the one chasing.

Screening calls, no-shows and follow-ups land on the desk of someone with a facility to run.

The work behind the title

What does this hire really come down to?

Your next CFO may review a cash forecast, question a service-line margin, and explain a capital request to the board on the same day. We recruit finance leaders who can show the source data, explain the tradeoff, and state what action they would recommend.

Tell us what needs fixing first. Delayed collections need different experience than debt refinancing. We ask which reports, contracts and spending decisions this CFO will own.

A system controller may know the reports without having owned contracts or capital spending. We ask what they decided themselves and who challenged them. Smaller-provider experience needs a separate check for hospital reporting.

Screening

What do we check before you see anyone?

We verify active CPA licensure when the candidate presents a CPA credential or your position calls for one. Since licensure is state-based, we confirm current status and ask how the executive stays current with GAAP and healthcare reporting changes. CHFP can show useful finance education, but it is not a universal condition for a CFO position.

  • We examine cost-report experience through a specific example. Which schedules did the executive review? How did they test source data from finance, reimbursement, or operations? We ask what an auditor questioned, how the candidate responded, and whether the issue changed an estimate, reserve, or reporting process.
  • We screen 340B experience when that program falls within the CFO’s scope. Candidates should describe their finance controls, pharmacy handoffs, reporting role, outside-review findings, and follow-up on identified exposure. This separates direct accountability from attendance at a committee meeting.
  • We test commercial payer contract modeling with a practical scenario. If a proposed rate increase improves one service line but changes terms elsewhere, what assumptions would the candidate model? We ask how value-based payment risk, volume, denials, and collection timing could alter the stated rate improvement.
  • We review cash, accounts receivable, and capital planning as connected decisions. A CFO should distinguish a short collections delay from a structural margin concern before deferring equipment, changing spending controls, or revising a forecast. We ask what data would support each conclusion.
  • We assess communication with clinical, operating, and board leaders through a difficult funding decision. Can the candidate explain why a request moved forward, changed scope, or paused without hiding behind finance language? You complete credentialing and any clinical assessment. We screen credentials, work history, leadership fit, and the candidate’s answers about actual finance work.
Sourcing

Where do we find them, and why would they move?

Our primary source pool includes controllers and VP finance leaders from hospitals, health systems, and other healthcare delivery organizations. Some may consider a move because their present seat limits board exposure, full balance-sheet responsibility, or influence over long-range capital decisions.

We also speak with finance executives from healthcare organizations with focused service lines. They may seek broader responsibility across payer strategy, reporting, capital planning, and operating decisions. We test the gap between their current scope and your CFO seat rather than treating adjacent healthcare experience as interchangeable.

Avoid these

Which mistakes stall the search?

A common mistake is selecting a polished presenter who cannot walk through a cost report, cash forecast, or payer model they personally reviewed. Another is treating a CPA or CHFP as proof of hospital finance judgment without examining the decisions behind the credential.

03

Waiting until the offer stage to introduce operating and clinical leaders, when a communication problem may emerge too late.

Role-specific search plan

How would we run the search for this opening?

We define the decisions awaiting your next CFO. We interview against your scope using examples involving cost reports, 340B oversight, payer contracts, service-line margin, cash, and capital. You meet candidates whose background fits the brief. You may need cash stabilization, contract analysis, lender reporting, capital planning, growth support, or several of these.

01

Set the CFO brief

We clarify reporting lines, board exposure, decision authority, and the work expected during the first year.

02

Screen financial judgment

We ask what data changed the candidate’s recommendation, who disagreed, and how the executive explained the tradeoff.

03

Run a focused selection process

We coordinate feedback, discuss reasons for a move and compensation, and support offer-stage communication. No upfront fee. You pay only if you hire.

Questions

How do you handle counteroffers when a CFO candidate seeks broader board responsibility?

We ask early why the person is open to leaving, such as limited scope, a leadership change, location, or a wish for broader responsibility. At offer stage, we revisit those reasons so you can judge whether the move still has a sound basis.

Do we need a CPA for this CFO search?

That depends on your governance structure, finance team, and assigned work. We can target active CPA candidates and verify presented licensure, or include leaders with other relevant backgrounds, including CHFP, when your brief supports that choice.

Can you recruit a CFO for a turnaround or growth plan?

Yes, but the screening focus changes. For cash pressure, we examine collections, expense decisions, lender communication, and revenue-cycle diagnosis. For growth, we review capital models, payer strategy, forecasting, and board communication.

How do you assess 340B experience?

We ask candidates to describe their finance role in program oversight, reporting, controls, and responses to review findings. Pharmacy leaders may own other parts of the program, so we ask where the CFO’s authority began and ended.

What are the search terms and fee structure?

No upfront fee. You pay only if you hire. We offer a candidate replacement guarantee on standard search agreements. The applicable period and exact terms are set out in the signed client recruiting agreement.

How long will it take to see a short list?

That depends on the job, schedule, available people, and what your employer can offer. We discuss those constraints before outreach and update you as candidates respond. We do not promise a fixed shortlist date. We check the job brief before arranging interviews. Tell us which parts of the role you can change.

What are the replacement terms?

We offer a candidate replacement guarantee on standard search agreements. The applicable period and exact terms are set out in the signed client recruiting agreement. Ask us to walk through those terms before you sign, so you know how a replacement would work for your opening and who would start it.

What do we not do in a standard permanent search?

We do not bill standard contingency search by the hour or simply repost your job. Temporary coverage is a separate arrangement. Any retainer or different engagement must be agreed, not assumed. Tell us early if you need coverage while the permanent search runs.
