What an open seat really costs

What does an open seat cost you every week?

An open role costs you more than the recruiting fee. Someone covers the work, waits for care, or stops a service. We ask what is falling behind and who is carrying it. Use your own figures to compare the cost of waiting with the cost of hiring.

01

The seat is costing you every week.

Overtime, diverted patients, a stretched team and a schedule held together by favors.

02

The posting brought the wrong people.

Applicants arrive, but the person who has done this exact work is employed and not looking.

03

You are the one chasing.

Screening calls, no-shows and follow-ups land on the desk of someone with a facility to run.

The work behind the title

What does this hire really come down to?

A cardiovascular, oncology, orthopedic, or neuroscience service line needs a leader who can read the monthly numbers and act on them. This person may review implant spend, prepare a capital request, meet a medical director, and track certification work in the same week.

We recruit for that direct operating record.

The strongest candidate can explain what they owned in a service line P&L. Ask where revenue was lost, which costs changed, and who had authority to approve the change. A title alone does not show whether the leader controlled a budget or simply received reports after decisions were made.

The administrator may need to bring surgeons, cardiologists, oncologists, or neurologists into a discussion about equipment, scheduling, growth plans, or supply variation. Ask for a disagreement they handled, the data used, the final decision, and what happened after the meeting.

Screening

What do we check before you see anyone?

Service line P&L ownership. We ask candidates to separate their direct budget responsibility from finance reporting they only reviewed. We explore volume, revenue capture, labor, supply expense, capital planning, and the choices they made when performance moved away from plan.

  • Physician alignment. We ask about committee work, co-management arrangements, and conversations with medical directors. A useful answer identifies the competing interests, such as surgeon preference versus cost, then explains how the candidate reached a workable decision.
  • Specialty program certifications. We screen for hands-on work with ACC, CoC, or TJC disease-specific programs when relevant to your opening. We ask who gathered evidence, ran mock reviews, corrected gaps, and spoke during the survey process.
  • High-cost supplies and equipment. For orthopedics, that may mean implants and vendor agreements. For cardiovascular services, it may include catheterization lab inventory. We ask how the candidate reviewed variation, involved physicians, and measured the financial effect of a change.
  • Capital requests. We ask candidates to walk through a request for imaging, robotics, procedure equipment, or another major purchase. They should explain demand assumptions, operating expense, projected return, alternatives considered, and the people who had to approve it.
  • Operating scope and team leadership. We confirm which sites, departments, managers, and physician groups sat within the candidate’s remit. We also ask how they handled a staffing, access, or throughput issue without shifting responsibility to another department.
Sourcing

Where do we find them, and why would they move?

We approach directors and executive leaders from cardiovascular, oncology, orthopedic, and neuroscience programs. Relevant backgrounds include integrated health systems, regional medical centers, and multi-site specialty programs where leaders report on departmental financial performance and work with physician leadership. We test the reason for change against your role, location, decision rights, and available resources rather than treating interest as commitment.

People consider a move for a wider service-line remit, authority over more sites, a clearer reporting structure, or a chance to build a program.

Avoid these

Which mistakes stall the search?

A common mistake is hiring a broad hospital administrator who cannot describe direct service-line P&L decisions, physician preference-item discussions, or specialty program work. Another is accepting survey exposure as certification leadership when the candidate cannot identify their own audit, corrective-action, or education duties.

03

Leaving decision rights vague, then interviewing candidates whose previous roles had far more or far less authority over physicians, capital, and budgets.

Role-specific search plan

How would we run the search for this opening?

Tell us the service line, reporting relationship, P&L scope, physician groups, certification status, and decisions this leader can make. We present candidates with relevant history and discuss the work behind it. You select who advances, interview finalists, and complete your own credentialing, references, and clinical assessment where applicable.

01

Set the operating brief

We turn those details into screening questions rather than searching on title alone.

02

Review proven decisions

If your team uses a case discussion, ask how the person would frame a supply, growth, or capital decision for physicians and executives.

03

Complete your review

No upfront fee. You pay only if you hire, under a written agreement that sets fee and replacement terms.

Questions

Can you help with interim or nonpermanent coverage?

We arrange locum tenens, travel and contract coverage on request. Credentialing, licensing and malpractice coverage are confirmed per engagement before anyone starts. Per diem placement means adding a person to your own pool, not covering individual shifts. Physician relationships need equal scrutiny.

Should we recruit one administrator for several specialties?

That depends on reporting scope and how much shared infrastructure the programs have. We will ask whether the leader owns one specialty deeply or coordinates cardiovascular, oncology, orthopedic, and neuroscience leaders across several sites.

How do you distinguish finance experience from P&L ownership?

We ask what budget lines the candidate controlled and what actions they personally approved or recommended. A candidate who only reviewed monthly reports is presented differently from one who changed supply, growth, or capital decisions.

Do candidates need ACC, CoC, or TJC disease-specific experience?

The right credential history depends on your program and its current status. We screen for relevant exposure and ask about specific survey duties, but your organization determines its own certification and credentialing standards.

What if physicians want a different leader than finance does?

Bring both viewpoints into the selection process early. We can screen for examples of physician committee work and financial discussions, then your interview panel can test how the finalist explains tradeoffs to each group.

Next step

What should you send us about this role?

Send us the role, where the person will work and what needs to change with this hire. We will ask about the work, not just the title. Share any license or schedule limits that could rule someone out before we begin the search.

If you need a leader who can account for specialty performance and earn physician participation, contact Healthcare Headhunters to discuss the role.

No upfront fee. You pay only if you hire. We offer a candidate replacement guarantee on standard search agreements. The applicable period and exact terms are set out in the signed client recruiting agreement.

How long will it take to see a short list?

That depends on the job, schedule, available people, and what your employer can offer. We discuss those constraints before outreach and update you as candidates respond. We do not promise a fixed shortlist date. We check the job brief before arranging interviews. Tell us which parts of the role you can change.

What if a candidate receives a counteroffer?

We ask why the person wants to move and discuss competing decisions before an offer. A counteroffer can still change their choice. Keep the duties and offer clear rather than assuming more money settles every concern. Tell us early if parts of your offer cannot change, so the candidate hears it from us first.

What are the replacement terms?

We offer a candidate replacement guarantee on standard search agreements. The applicable period and exact terms are set out in the signed client recruiting agreement. Ask us to walk through those terms before you sign, so you know how a replacement would work for your opening and who would start it.

What do we not do in a standard permanent search?

We do not bill standard contingency search by the hour or simply repost your job. Temporary coverage is a separate arrangement. Any retainer or different engagement must be agreed, not assumed. Tell us early if you need coverage while the permanent search runs.
