What an open seat really costs

What does an open seat cost you every week?

An open role costs you more than the recruiting fee. Someone covers the work, waits for care, or stops a service. We ask what is falling behind and who is carrying it. Use your own figures to compare the cost of waiting with the cost of hiring.

01

The seat is costing you every week.

Overtime, diverted patients, a stretched team and a schedule held together by favors.

02

The posting brought the wrong people.

Applicants arrive, but the person who has done this exact work is employed and not looking.

03

You are the one chasing.

Screening calls, no-shows and follow-ups land on the desk of someone with a facility to run.

The work behind the title

What does this hire really come down to?

A Healthcare CIO or VP of Information Technology directs enterprise systems, infrastructure, security, vendors, and technology spending. In one week, this leader may review an outage, challenge a renewal quote, meet clinical leaders about an EHR change, and explain a risk decision to the board.

First, decide whether you need a builder or a stabilizer. A builder may lead an EHR replacement, acquisition integration, cloud move, or digital service launch. A stabilizer may fit better when outages recur, hardware is aging, ownership is unclear, or vendors leave unresolved problems.

The title changes the scope. A CIO commonly owns enterprise strategy and board reporting. A VP of Information Technology may lead applications, infrastructure, security, or operations under a CIO. A Chief Medical Information Officer often leads physician participation in clinical system design rather than all technology functions.

Screening

What do we check before you see anyone?

Ask about direct HIPAA and HITECH work. We ask how they handled a privacy concern, reviewed access, briefed leadership, and changed a process afterward. Useful answers name the records reviewed, decision owners, and limits of their authority. Match their background to your EHR and connected tools.

  • We ask about upgrades, data conversion, interface failures, and documentation changes. Platform familiarity matters, but so do testing changes and responding when registration, laboratory, imaging, billing, or clinicians report defects.
  • Probe vendor management through a service failure. We ask how they read service-level terms, documented missed performance, escalated an outage, and chose between renewal, renegotiation, or replacement. They should distinguish a vendor promise from the organization’s recovery plan.
  • Test cloud and on-premise judgment. We ask what workload they moved, what data and access questions they reviewed, and how they compared subscription charges, hardware refreshes, staffing, and downtime exposure. Neither hosting model fits every organization.
  • Review budget decisions. Ask for an example of retiring a duplicate tool, delaying a project, or funding a security control while protecting an application upgrade. Look for what was deferred, who approved it, and how the candidate explained the consequence.
  • Check clinical engagement without treating this as a bedside role. We ask how they gathered feedback before changing a workflow and what they did when users disagreed. Strong leaders observe registration, documentation, reporting, and handoff work instead of relying only on dashboard reports.
Sourcing

Where do we find them, and why would they move?

We recruit from academic medical centers, multi-hospital health systems, regional outpatient networks, and other provider organizations. Some leaders have completed a consolidation, upgrade, or infrastructure program and seek broader enterprise authority or a different operating model. Others may consider a move because their current scope covers one campus, one technology domain, or maintenance work.

We discuss their reason for moving, reporting expectations, relocation, compensation, and interest in the decisions your role will own.

Avoid these

Which mistakes stall the search?

A common mistake is hiring an impressive technical résumé without testing how the candidate explains downtime, cyber risk, or spending choices to nontechnical executives. Another is treating an EHR certificate as proof of vendor, budget, security, infrastructure, and cross-department leadership judgment.

03

Interviewing only executives and excluding leaders who deal with access, documentation, interfaces, reporting, and registration problems.

Role-specific search plan

How would we run the search for this opening?

List failures in plain language. State whether the hire owns enterprise strategy, applications, infrastructure, security, or a defined program. Give finalists a hypothetical situation, such as an EHR outage during a peak clinic day or a renewal after repeated vendor failures.

01

Document the work to be fixed

Include slow logins, recurring downtime, broken laboratory or imaging interfaces, duplicate entry, security findings, and contracts nearing renewal. Identify the current owner and the effect on staff work.

02

Set scope and authority

Clarify reporting lines, budget authority, board exposure, clinical governance work, and decisions reserved for the CEO or board.

03

Interview against real decisions

Ask what they would learn first, who they would involve, what they would defer, and how they would report progress.

Questions

How does your fee structure work?

No upfront fee. You pay only if you hire. We offer a candidate replacement guarantee on standard search agreements. The applicable period and exact terms are set out in the signed client recruiting agreement. We arrange locum tenens, travel and contract coverage on request. Credentialing, licensing and malpractice coverage are confirmed per engagement before anyone starts.

How long should a CIO or VP of IT search take?

Timing depends on scope, location, compensation, reporting structure, and whether direct experience with a particular EHR matters. We address those choices early so interviews focus on people whose background fits the work.

Should we ask for a specific EHR certificate?

A platform certificate can help for a hands-on applications leader. It is not a universal legal mandate or a substitute for enterprise governance, vendor management, security judgment, financial oversight, and clinical stakeholder work.

How do we assess someone from a different provider setting?

Ask about comparable decisions rather than matching titles. A leader from a multi-site outpatient network may bring relevant experience with access management, interfaces, vendor contracts, and growth planning if your role involves similar work.

How should we handle a counteroffer after discussing enterprise authority and reporting access?

Counteroffers can arise when a leader holds deep institutional knowledge. We ask early why they would consider moving and revisit those reasons before final interviews and an offer, including scope, reporting access, and unresolved work.

Next step

What should you send us about this role?

Send us the role, where the person will work and what needs to change with this hire. We will ask about the work, not just the title. Share any license or schedule limits that could rule someone out before we begin the search.

Tell us what is failing, what authority the leader will hold, and which decisions need attention first. We will screen Healthcare CIO and VP candidates against that work.

What are the replacement terms?

We offer a candidate replacement guarantee on standard search agreements. The applicable period and exact terms are set out in the signed client recruiting agreement. Ask us to walk through those terms before you sign, so you know how a replacement would work for your opening and who would start it.

What do we not do in a standard permanent search?

We do not bill standard contingency search by the hour or simply repost your job. Temporary coverage is a separate arrangement. Any retainer or different engagement must be agreed, not assumed. Tell us early if you need coverage while the permanent search runs.
