What an open seat really costs

What does an open seat cost you every week?

An open role costs you more than the recruiting fee. Someone covers the work, waits for care, or stops a service. We ask what is falling behind and who is carrying it. Use your own figures to compare the cost of waiting with the cost of hiring.

01

The seat is costing you every week.

Overtime, diverted patients, a stretched team and a schedule held together by favors.

02

The posting brought the wrong people.

Applicants arrive, but the person who has done this exact work is employed and not looking.

03

You are the one chasing.

Screening calls, no-shows and follow-ups land on the desk of someone with a facility to run.

The work behind the title

What does this hire really come down to?

An intensivist search starts with how your ICU operates, not the title alone. We ask who admits patients, who directs critical care, and who responds overnight. Healthcare Headhunters recruits for permanent positions, with screening built around the clinical responsibilities and working relationships you expect this physician to carry.

Define whether your ICU is open, closed, or organized around mandatory consultation. Then describe the intensivist’s authority over orders, transfers, and discharge decisions. If admitting physicians retain responsibility, candidates need a clear account of how disagreements are resolved. Otherwise, both sides may enter interviews with different expectations about who directs care.

Look beyond fellowship labels when assessing fit. Ask which patients they personally managed, which procedures they performed, and which specialists were available. Your hospital completes credentialing and clinical assessment; we screen credentials, work history, and fit through concrete questions.

Screening

What do we check before you see anyone?

License and training: Which state medical licenses do you hold, and what was your critical care training pathway? We review reported license status, fellowship history, and board certification or eligibility against your medical staff criteria. Specialty certification does not replace the required state medical license.

  • Shift responsibility: On your last ICU service, who handled admissions, bedside emergencies, transfers, and overnight calls? A useful answer separates personal responsibility from resident, fellow, advanced practice clinician, and consulting coverage. Ask how responsibility changed after the daytime team left.
  • Clinical judgment: Describe a deteriorating patient whose initial explanation proved incomplete. What changed your assessment, and when did you involve another service? Look for a coherent account of reassessment, communication, and escalation rather than a list of diagnoses or procedures.
  • Procedural scope: Which ICU procedures have you performed recently, and which did another service cover? Ask about procedural records, supervision, and any practice gaps. Compare the answers with your proposed privileges and backup arrangements rather than assuming all intensivists have identical procedural experience.
  • Team communication: How do you conduct rounds when nursing concerns differ from the initial medical plan? Ask for an example showing how bedside observations were heard, decisions were communicated, and unresolved issues reached the appropriate physician. Clarify expectations for teaching if trainees participate.
  • Family discussions and handoffs: How do you explain uncertainty to families and transfer responsibility at shift change? Useful answers identify outstanding decisions, pending results, escalation plans, and documented discussions. Ask how the physician handles disagreement without leaving the incoming team to reconstruct the conversation.
Sourcing

Where do we find them, and why would they move?

We look at physicians from community hospitals, academic medical centers, regional referral hospitals, and multispecialty groups with critical care services. The source setting helps frame questions, but it does not establish fit. A referral-center physician may have relied on specialist backup that your hospital does not provide.

Possible reasons for a move include a different call schedule, more direct patient care, a teaching role, or relocation. We ask candidates what they want to change and what they want to retain. That distinction helps you discuss the actual position instead of selling an ICU arrangement they would rather leave.

Avoid these

Which mistakes stall the search?

A common mistake is advertising daytime ICU coverage without describing nights. State whether the physician takes home call, returns onsite, supervises another clinician, or shares coverage with another intensivist. Another is treating procedure lists as proof of current practice. Ask when procedures were last performed, what documentation is available, and whether additional review would be needed before privileges are granted.

03

Leaving authority unclear between intensivists and admitting services. Describe who writes orders and leads family discussions before interviews, especially if your hospital is changing its ICU model.

Role-specific search plan

How would we run the search for this opening?

We review the ICU model, patient mix, schedule, procedural expectations, and available specialist support with you. We discuss recent ICU assignments, license status, training, clinical responsibilities, and reasons for moving. You meet candidates with a shared description of coverage and authority.

01

Define the service

We also clarify teaching duties and committee work so the search reflects the full position.

02

Screen against the work

We present relevant experience alongside questions that still need your clinical interview or credentialing review.

03

Test the fit

Use interviews to examine clinical judgment and team relationships, then complete your medical staff review before setting a start date.

Questions

Should pulmonary practice be part of this position?

Only if you want outpatient pulmonary visits, consults, or related duties included. State their share of the schedule; not every critical care physician has the same training background. A physician’s recent practice may center on medical, surgical, cardiac, or mixed critical care.

How should we describe overnight responsibility?

Name who is physically present and who responds remotely. Explain escalation routes and whether the intensivist must return to the hospital.

What if we have limited specialist backup?

Describe what is available onsite and how transfers work. Ask candidates which decisions they handled independently in comparable circumstances.

Can an intensivist also supervise advanced practice clinicians?

Discuss the actual supervision duties and coverage arrangement. Ask about experience reviewing cases, giving feedback, and handling escalation during overnight coverage.

How do we assess teaching fit?

Explain who learns on rounds and how teaching affects the day. Ask candidates how they balance instruction with time-sensitive patient decisions.

Next step

What should you send us about this role?

Send us the role, where the person will work and what needs to change with this hire. We will ask about the work, not just the title. Share any license or schedule limits that could rule someone out before we begin the search.

Tell us how your ICU is staffed and where physician coverage needs to change. We can screen for experience directing ICU admissions, sharing overnight responsibility, and working with your available specialist backup.

No upfront fee. You pay only if you hire. We offer a candidate replacement guarantee on standard search agreements. The applicable period and exact terms are set out in the signed client recruiting agreement.

How long will it take to see a short list?

That depends on the job, schedule, available people, and what your employer can offer. We discuss those constraints before outreach and update you as candidates respond. We do not promise a fixed shortlist date. We check the job brief before arranging interviews. Tell us which parts of the role you can change.

What if a candidate receives a counteroffer?

We ask why the person wants to move and discuss competing decisions before an offer. A counteroffer can still change their choice. Keep the duties and offer clear rather than assuming more money settles every concern. Tell us early if parts of your offer cannot change, so the candidate hears it from us first.

What are the replacement terms?

We offer a candidate replacement guarantee on standard search agreements. The applicable period and exact terms are set out in the signed client recruiting agreement. Ask us to walk through those terms before you sign, so you know how a replacement would work for your opening and who would start it.

What do we not do in a standard permanent search?

We do not bill standard contingency search by the hour or simply repost your job. Temporary coverage is a separate arrangement. Any retainer or different engagement must be agreed, not assumed. Tell us early if you need coverage while the permanent search runs.
