- Director of Care Management
- Case Management Manager
What does this hire really come down to?
A Director of Case Management directs discharge planning, payer review, social work oversight, and avoidable-day follow-up. You need a leader who can review why a patient remains, assign the next action, and bring unresolved barriers to the right discussion. We screen for leaders who have managed staff, reporting, and daily discharge decisions.
This is not a paperwork-only leadership job. During a shift, the director may review patients without a discharge date, question vague barriers in case notes, review denial themes, and decide where manager intervention is needed. Strong candidates can describe what they changed, who owned each follow-up, and how they tracked the effect.
Test judgment when parties disagree. A physician may consider a patient ready while family decisions, payer review, transport, or a receiving setting delays the transition. The director does not direct treatment. They organize the case management response, clarify documentation questions, escalate the right issue, and keep case management and social work assignments clear.
What do we ask candidates about?
- Ask whether the candidate holds ACM or CCM certification. Either credential can show continued engagement with case management practice, but it does not replace your credentialing review. We ask how the candidate uses that knowledge during difficult discharge planning discussions.
- Review length-of-stay and avoidable-day work in detail. We ask for one recurring delay, such as late referrals or unclear ownership after rounds. Then we ask what report they reviewed, what action changed, and what they examined the next week.
- Test oversight of discharge planning and social work. Ask how the candidate assigns complex cases, reviews barriers, and decides when a social worker needs manager support. Useful answers separate clinical barriers, family choices, payer questions, and placement delays.
- Discuss caseload management. We ask how the leader identifies overdue assessments, pending referrals, and uneven assignments. We also ask what they do when a high-risk transition has no documented next step before the day ends.
- Explore denial tracking and documentation follow-up. Candidates should explain how findings reach physician advisors, revenue cycle colleagues, or documentation leaders. Listen for a defined handoff and an informed response to recurring themes, rather than blame toward nursing staff.
- Ask about post-acute coordination. A referral accepted on paper can still face authorization, transport, family, or bed-timing barriers. We ask how candidates review pending options, document outreach, and escalate a stalled transition.
We discuss experience and qualifications. Your team assesses clinical ability and completes the credential checks before hiring.
Where do we look, and why would they move?
Our source pool includes case management managers and UR managers. Some seek a director role because their current authority stops at daily reviews. Others want responsibility for staff development, reporting, and department decisions.
People may also consider a move after leadership changes, limited authority over discharge processes, or a desire to lead a broader team. We discuss why they are open to a change and whether your reporting structure, decision rights, and present barriers fit their goals.
Which mistakes stall the search?
Hiring a strong individual case manager who has not managed assignments, social work performance, or recurring delay reviews.
Accepting broad claims about length-of-stay improvement without asking which avoidable days the candidate owned and what changed.
Interviewing only for physician relationships while skipping payer review, referral follow-up, denial themes, and staff accountability.

What do employers ask us?
Should ACM or CCM certification be mandatory for this role?
That depends on your policy and the scope you assign to the director. ACM or CCM may be a useful preference, but certification alone does not prove leadership over avoidable days, staff, or discharge operations.
How do we test whether someone has truly improved length of stay?
Ask for one defined delay, the measure reviewed, the action taken, and the result observed. Follow with what did not work, because candidates with direct ownership can usually explain how they adjusted their approach.
What should we ask about social work oversight?
Ask how the candidate reviews caseloads, responds to missed follow-up, and coaches staff on family and referral documentation. Also ask how they distinguish a staff performance issue from a barrier outside the hospital's control.
Can Healthcare Headhunters recruit for permanent or other coverage needs?
Permanent/direct-hire recruitment is our core service across clinical, operational and leadership roles. If you need a temporary or other arrangement, describe the need so we can discuss whether we can help. Responsibilities and terms must be confirmed separately for that engagement. Your organization retains clinical assessment and credentialing responsibility.
