The work behind the title

What does this hire really come down to?

A Director of Clinical Services connects office review with the work completed in patients’ homes. The right person reviews documentation, guides field clinicians, tracks quality signals, and addresses recurring problems before they become agency-wide patterns.

This is not simply a senior nursing hire. You need someone who can read a clinical record, ask why a visit was late or incomplete, and decide whether the issue is training, workload, supervision, or follow-through. Ask how they turn a finding into a specific conversation with the clinician and a documented follow-up check.

Consider the authority attached to the role before opening a search. A director cannot correct repeat documentation problems if ownership or operations reverses every clinical decision. Define who reviews their findings, who handles performance action, and what access they have to quality, billing, and intake information.

Experience that matters

What do we ask candidates about?

  • Active licensure and role scope: Confirm an active RN license when your state and agency role call for one. We ask which clinical decisions they personally owned and where another leader or medical provider made the final decision.
  • Chart review method: Ask the candidate to describe a recent internal review from first finding to recheck. Look for a clear process for comparing patient status, visit notes, OASIS data, and the plan of care.
  • Survey participation: Explore direct work with CHAP or TJC surveys, if applicable to your agency. Ask what records they assembled, how they briefed staff, and how they tracked correction work after survey findings.
  • Quality measures and coaching: Ask how they use star ratings and other quality results to identify a coaching topic. Strong answers name the report, the group affected, the teaching action, and the later measure they reviewed.
  • Remote field oversight: Discuss how they monitor visit compliance across rural or dispersed territories. Ask what they do when a clinician misses a visit pattern, cannot complete documentation, or does not respond to repeated outreach.
  • Clinical software knowledge: Verify experience with your electronic health record system or a comparable platform. Ask how they found unsigned orders, incomplete notes, or documentation holding up billing, rather than accepting a general claim of software skill.

We discuss experience and qualifications. Your team assesses clinical ability and completes the credential checks before hiring.

Finding people

Where do we look, and why would they move?

We recruit from independent home health agencies, hospital-affiliated post-acute programs, and other employers with distributed clinical teams. Candidates may move when they want clearer decision rights, a smaller reporting structure, or a chance to repair a recurring quality problem rather than only report it.

A hospital-based leader may bring useful audit discipline, but home health calls for different supervision habits. We ask how they supported staff working alone in the field and how they connected office findings to actual visit documentation.

Avoid these

Which mistakes stall the search?

01

Do not hire based on years in home health alone without asking the candidate to explain a chart-review finding and the action that followed.

02

Do not treat CHAP or TJC exposure as proof of leadership when the candidate only attended meetings or supplied records for another leader.

03

Do not leave authority vague and then expect the director to correct repeat late notes or missed coaching follow-up without management backing.

Questions

What do employers ask us?

Should this director already know our exact electronic health record?

Exact platform experience can shorten orientation, but it should not outweigh sound chart-review judgment. Ask how the candidate used prior systems to locate late documentation, unsigned orders, and reporting trends.

Can a hospital clinical leader succeed in home health?

Possibly, if they have supervised remote staff and can explain home-based documentation review. Test how they would follow a concern from a field note through coaching, correction, and later review.

What should we ask about audit performance?

Ask for one example of a discrepancy between patient status and the record. The candidate should explain how they confirmed the facts, assigned follow-up, and checked whether the problem recurred.

How do we handle a candidate considering another offer or a counteroffer?

Discuss the actual work they would own here, including authority, reporting access, and the problems they would be expected to address. A counteroffer may change pay, while unresolved limits on decision-making or resources can remain.

What are the engagement terms for this search?

Permanent/direct-hire recruitment is our core service across clinical, operational and leadership roles. If you need a temporary or other arrangement, describe the need so we can discuss whether we can help. Responsibilities and terms must be confirmed separately for that engagement. Your organization retains clinical assessment and credentialing responsibility.
